Institution Evaluation

Add an Institution Evaluation

1. Click Add an Institution Evaluation.
2. Select eForm as the evaluation type.
3. Select your dental school. If your school is not listed, select Other. Verify that you are selecting the correct schools - ADEA PASS will send the IEF to the school selected.
4. Select your graduation year.
5. The dental school name, evaluator name, and evaluator email address populate automatically.
6. Select a due date. This should be your earliest program deadline.
7. Indicate your agreement with the right of access statement. This selection cannot be changed after submission.
8. Click Save and Continue.

Request an Exception

If you attended a non-US dental school and cannot obtain this document, or your dental school has closed, you must request an exception. The request and explanation will then be processed and distributed to each of your programs. Note that this exemption only waives the requirement for ADEA PASS application. Individual programs may require additional documentation in lieu of the IEF. Contact your programs directly for more information.

1. Click Add an Institution Evaluation.
2. Select Request for Exception as the evaluation type.
3. Select your dental school. If your school is not listed, select Other.
4. Select your reason for exception.
5. Enter a brief explanation of why you are requesting the exception.
6. Click Save and Continue.
