A Significant Concern is entered by the CI when a student has an issue with one of the criteria for the APTA Clinical Performance Instruments. Concerns need to be entered as part of the midterm or final CP and must be accompanied by a Critical Incident Report using APTA Clinical Performance Instruments.
To submit a significant concern, select the Significant Concern checkbox.
A confirmation message appears. If you click OK, the Critical Incident Report text boxes appear. Follow the steps above to create and submit the Critical Incident Report.
Note: If the Critical Incident Report is not submitted, the Significant Concern will not be submitted. You cannot reverse a Significant Concern and accompanying Critical Incident Report once they are submitted.
Click Cancel to prevent submitting the Significant Concern.
