Evaluation Forms

You can view and edit all components of evaluation forms used in the Supporting Information and Program Materials sections, including enabling or disabling the evaluation form and editing evaluation instructions, questions, rubric ratings, and document uploads. You can choose to activate:

  • CAS-level evaluations, which are listed in the Supporting Information section. These include options for individual and committee letter evaluations. If you choose this option, only one evaluation form can be activated.
  • Program-level evaluations, which are listed in the Program Materials section. These are configured by the programs in the Configuration Portal. If you choose this option, you can create as many evaluation forms as needed.

Note that both CAS- and program-level evaluations can't be activated at the same time in the same CAS and cycle.

To add a new form:

  1. Click Add Form Type.
  2. Enter a name.
  3. Click Save.

You can copy existing forms by clicking Copy. All instructions, questions, document uploads, and rubric criteria carry forward. You must name the new evaluation form before completing the copy. You can also delete existing forms by clicking Delete.

You can then click the evaluation form name to configure the evaluation forms type.

Form Types

To edit the form name:

  1. Click the edit icon.
  2. Enter the new name.
  3. Click the check icon.

If you want to edit the instructional text to this form:

  1. Click the edit icon.
  2. Enter your text. You can use the formatting options to further customize your text.
  3. Click Save.

You can view the evaluation form blocks. Select which evaluation form blocks you want to have active. You can reorder them by dragging and dropping.

Upload Letter

In this evaluation form block, you can select if evaluators can upload a file to the evaluation form.

To edit the tile name:

  1. Click the edit icon.
  2. Enter the new name.
  3. Click the check icon.

If you want to edit the instructional text to this tile:

  1. Click Edit.
  2. Enter your text. You can use the formatting options to further customize your text.
  3. Click Save.

Note that you can add only one question, a file upload, to this tile.

Add a File Upload
  1. Click Add Question.
  2. Enter the question. You can use the formatting options to further customize your text.
  3. Select if the question is required.
  4. Select the answer type File Upload.
  5. Click Save & Close.

Once your question is added, you can:

  • Edit it by clicking the edit icon.
  • Delete it by clicking the trash icon.

Overall Recommendation

In this evaluation form block, you can add a question which evaluators can only select one answer. For example:

  • Question: How long have you known this applicant?
    • Answers: less than a year; over 1 year; over 5 years, etc.

To edit the tile name:

  1. Click the edit icon.
  2. Enter the new name.
  3. Click the check icon.

If you want to edit the instructional text to this tile:

  1. Click Edit.
  2. Enter your text. You can use the formatting options to further customize your text.
  3. Click Save.

Note that you can add only one question, a drop-down, to this tile.

  1. Click Add Question.
  2. Enter the question. You can use the formatting options to further customize your text.
  3. Select if the question is required.
  4. Select the answer type Dropdown.
  5. Enter the answer option values.
  6. Click Add Option to add additional options, if necessary. Click the delete icon to remove any answers.
  7. Click Save & Close.

Once your question is added, you can:

  • Edit it by clicking the edit icon.
  • Delete it by clicking the trash icon.

Relationship

In this evaluation form block, you can add questions about how an evaluator knows the applicant. For example:

  • In what capacity have you worked with this applicant?

To edit the tile name:

  1. Click the edit icon.
  2. Enter the new name.
  3. Click the checkmark icon.

If you want to edit the instructional text to this tile:

  1. Click Edit.
  2. Enter your text. You can use the formatting options to further customize your text.
  3. Click Save.

You can add several questions to this question block.

Add Questions
  1. Click Add Question.
  2. Enter the question. You can use the formatting options to further customize your text.
  3. Select if the question is required.
  4. Select the answer type.
    • Dropdown: questions are single answer, where evaluators can choose only one response.
      1. Enter the answer option values.
      2. Click Add Option to add additional options, if necessary. Click the delete icon to remove any answers.
    • Radio: questions appear with radio buttons where evaluators can choose from two responses.
      1. Enter the answer option values.
    • Text: questions appear with a text box where evaluators can type their response. This displays as a small answer field.
      1. Enter the maximum characters an evaluator can enter. This includes spaces and punctuation.
    • Essay: questions appear with a text box where evaluators can type their response. This displays as a large answer field.
      1. Enter the maximum characters an evaluator can enter. This includes spaces and punctuation.
  5. Click Add Another Question. If finished, click Save & Close.

Once your question is added, you can:

  • Edit it by clicking the edit icon.
  • Delete it by clicking the trash icon.
  • Reorder your questions within the Relationship question set by dragging and dropping.

Rubrics

In this evaluation form block, you can create a rubric for evaluators to rank applicants on. Note that this may have a different label for each CAS (e.g., Likert Criteria, Evaluation Ratings, etc.). For example:

  • Rank the applicant on a scale of 1 to 5 for:
    • Intellectual Ability
    • Leadership
    • Team Skills

To edit the tile name and instructional text:

  1. Click the edit icon.
  2. Enter the new name.
  3. Enter your instruction text. You can use the formatting options to further customize your text.
  4. Enter custom names and aliases for each rating scale. Note that all rating fields must be completed.
  5. Click Save.

If you want to edit the instructional text to this tile:

  1. Click the edit icon.
  2. Enter your text. You can use the formatting options to further customize your text.
  3. Click Save.

You can add several criteria to this question block.

Add Criteria
  1. Click Add Criterion.
  2. Enter the criterion name.
  3. Click Save.

Once your criterion is added, you can:

  • Edit it by clicking the edit icon.
  • Delete it by clicking the trash icon.
  • Reorder your criteria within the Rubrics question set by dragging and dropping.
