Your Legal Name

Previous Name

If you have ever used a different legal name on any official documents or school transcripts than how your name appears on your application, select Yes in this section and enter your previous name.

Examples of previous names include:

  • Birth names (if different from current legal name)
  • Maiden names
  • Nicknames (e.g., "Bob" for Robert, "Beth" for Elizabeth, etc.)
  • "Westernized" names
  • Alternate punctuation (e.g., your name appears with/without a hyphen, apostrophe, or diacritical marking)
  • Alternate spacing (e.g., your name appears with or without specific spaces)
  • Known misspellings (if a document contains a misspelling)

If you commonly use a first or middle name that differs from your legal name, select Yes in this section and enter the information.

Gender and Sexual Orientation

This section asks about your gender and sexual orientation. Your responses help us better understand our applicants and students and are used to support student programs and services. This information will not be used or considered in the admission process. You may opt out of responding to any of these questions for any reason.

  • Gender refers to your gender identity and how you view yourself. Each person's experience with their gender identity is unique and personal. Gender identity may or may not differ from gender expression, which describes how we present ourselves to the world. While intersex is a biological sex term, it has been included with gender for inclusivity purposes.
  • Transgender or "trans" people's gender(s) differ from the gender they were assigned at birth. Transgender may include many different gender identities. People with nonbinary and gender-expansive identities may or may not identify as transgender.
  • Sexual orientation is a person's attraction to people of specific genders, which may or may not include sexual, romantic, and/or emotional attraction.
