Submitting a Significant Concern

A Significant Concern is entered by the CI when a student has an issue on one of the criterion for the CPI. Concerns need to be entered as part of the midterm or final CP and must be accompanied by a Critical Incident Report using CPI Web.

To submit a significant concern, select the Significant Concern checkbox.

A confirmation message appears. If you click OK, the Critical Incident Report text boxes appear. Follow the steps above to create and submit the Critical Incident Report

Note: If the Critical Incident Report is not submitted, the Significant Concern will not be submitted. You cannot reverse a Significant Concern and accompanying Critical Incident Report once they are submitted.

Click Cancel to prevent submitting the Significant Concern.
