This page applies to [CAS NAME] faculty members.
Your [trainee]'s institution can require forms and evaluations that require your review and signature. Once a form is submitted for your review, you will receive an email notification. Click the link in the email or log in to [CAS NAME] to access the form.
View Form and Evaluation Requests
You can view all the [placement]s that your [trainee]s created, with each supervisor listed on a separate row. You can also view all your form and evaluation requests.
- Click Forms.
- Click Submissions.
- Use the filter options to narrow your results, if needed.
- Click the details icon to the right of the [placement] row.
- Click view to review the form.
- Click resend to notify the recipient via email that they have a pending form.
- Click the trash icon to delete the form or evaluation request.
Complete a Form or Evaluation
- Click Forms.
- Click My Forms.
- Click Start Form.
- Complete all required fields.
- Click Complete "Faculty Member" Step to send the completed form to the next recipient or click Save to save your progress and return later.
- Click Print to print a hardcopy for yourself once your form is sent.
Edit a Form or Evaluation
You can edit forms you already submitted. If the program approved the form, you must contact them first to unapprove it in order to make changes.
- Click Forms.
- Click My Forms.
- Click View Submission.
- Click Unsubmit And Edit, then confirm that you want to reopen the form.
- Update fields, as needed.
- Click Complete "Faculty Member" Step to send the completed form to the next recipient or click Save to save your progress and return later.
- Click Print to print a hardcopy for yourself once your form is sent.
