Indicate whether you have ever been disciplined or placed on academic probation while attending an academic institution.

Indicate if you are a current or former UNT student. If you select Yes, enter your 8-digit student ID number. If you are a current student, indicate if you intend to change your current major.

Indicate your preferred location to attend classes.

Indicate if you plan to enroll full-time or part-time.
