ADEA CAAPID Agreement Statements
Review the statement carefully. This statement serves as a contractual agreement between you and ADEA CAAPID. You must agree to these terms in order to submit your application.
Release Statement
Click the checkbox to confirm that you give permission for ADEA CAAPID to release your application materials to the programs that you designate. Each program may use your application information for admission purposes and otherwise in accordance with such program's policies. For information regarding a program's policies, see the program's website or contact the program directly. To the fullest extent permitted by applicable law, ADEA CAAPID excludes all liability arising from the use of your information by any program.
Pre-Submission Release of Name and Contact Information
Selecting Yes for this release authorizes ADEA CAAPID to release your legal name and contact information to your selected programs before you submit your application. Your programs will be able to send you important information about their admissions process before you complete your application.
Notice of Matriculation Status
If you matriculate to a program, ADEA CAAPID will share your matriculation status with other programs that you applied to. Select the checkbox to indicate your understanding.
