Licensure and Certifications

In this section, you can add any current professional licensure or certifications that you have. Note that we do not verify the information entered here. Contact your individual programs to determine if they require official documentation of any licensure or certification listed in this section.

Add a Licensure

  1. Click Add a Licensure or Certification.
  2. Select Licensure from the Type drop-down.
  3. Select an option from the Type of License drop-down.
  4. Enter the name of the organization that issued the license.
  5. Enter the date on which the license was issued and the date it is valid to, if applicable.
  6. Select the state the license was issued in.
  7. Select if your licensure status is in good standing.

Add a Certification

  1. Click Add a Licensure or Certification.
  2. Select Certification from the Type drop-down.
  3. Enter the type of certification.
  4. Enter the name of the organization that issued the certification.
  5. Enter an optional, brief description.
  6. Enter the date the certification is valid to, if applicable.

Note that once you submit your application, you cannot edit or remove licenses and certifications; however, you can continue to add new ones, as needed.
