Pharmacy School Information

1. Enter the name of your terminal pharmacy school. This is the institution where you earned or will earn your most advanced pharmacy degree. This institution must be listed on the Colleges Attended page.
2. Indicate if this institution calculates a GPA.
   • If you select Yes, enter your GPA.
   • If you select No, select another performance-based indicator provided by the institution. If applicable, enter the related achievement or status. Or, select None of the above indicators are provided by my institution.
3. Click Save & Continue.
