Pharmacy Residency Program Information
Indicate if you are applying for postgraduate year 1 (PGY1) or postgraduate year 2 (PGY2). This section populates with the information you entered during account creation, but you can edit it before submission. Once you submit your application, you cannot edit this section.
Pharmacy School of Graduation
- Select the accredited pharmacy school you are currently attending OR previously attended. If your school is foreign and does not appear on the list and/or you selected ZZ - Foreign School (Non-ACPE), your PhORCAS application will be suspended. Click the checkbox to acknowledge the suspension and log out. PhORCAS will further review your application to determine eligibility. See Suspended Applicants/Residency Exemption Process for more information.
- Select your most advanced pharmacy degree.
- Indicate when you earned your most advanced pharmacy degree.
Pharmacist License Information
This section only applies to PGY2 program applicants. You must be licensed as a US Pharmacist to be considered for any of the US ASHP PGY2 programs. You are not eligible to apply for US PGY2 programs without this license.
- Select the state where you received your first US pharmacy license.
- Enter your license number.
- If you're licensed in multiple states, enter additional states and licenses in the field provided.
PGY1 Residency Program Information
This section only applies to PGY2 program applicants. If your PGY1 pharmacy residency program is not accredited or in candidate status with ASHP, you cannot apply to a PGY2 pharmacy residency program.
- Indicate if you completed or are in the process of completing a PGY1 residency program in the US. You cannot apply to a PGY2 pharmacy residency program if you do not complete an accredited PGY1 program in the US.
- Indicate when you completed or will complete your PGY1 program. To participate in the Match for a position that begins training at the PGY2 level, you must have already completed a PGY1 residency or currently be in training at a PGY1 residency that will be completed before the start of the PGY2 residency.
- Select the state your PGY1 program is located in.
- Select the PGY1 residency program. Note that PGY1 residency programs must be completed at an organization that is ASHP-accredited or in candidate status. If your residency program does not appear on this list and/or you select ZZ - Not Listed, your PhORCAS application will be suspended. Click the checkbox to acknowledge the suspension and log out. PhORCAS will further review your application to determine eligibility. See Suspended Applicants/Residency Exemption Process for more information.
