Building an Inquiry Form

Inquiry Form Overview

The Inquiry Form gathers information from students interested in your program or school. It includes fields for personal details (such as name, email, and phone number) and specific questions (such as major, sports, and extracurricular activities) to better understand their interests and needs.

When a student submits the Inquiry Form, they gain access to their PURL and can be added to various marketing campaigns. Only students with an Inquiry stage or higher can access their PURL. Prospective students can be contacted but are encouraged to complete the Inquiry Form to provide the necessary information for PURL creation.

Each {helpcenter} instance includes a single Inquiry Form, as multiple Inquiry Forms cannot be created per instance. For additional marketing campaigns or data collection, use SEM or Custom Forms.

Creating an Inquiry Form 

  1. Click Content in the top navigation bar and select Forms from the drop-down menu.
  2. Click New Forms and select Inquiry Form

Form Settings

  1. Input the General information. Note the following:
    • Use the Description to provide an internal explanation of how the form is being used.
    • The Header text is the bolded tag line at the top of the inquiry form.
    • The Sub-header text is normal font below the tag line typically used to provide instruction on completing the form.
  2. Click Form Access to choose which permission groups can access the form by checking the boxes.
  3. Click Inquiry Form and select your form editor:
    • Use Classic Layout keeps the form background white and allows you to add custom side banners.
    • Use Dynamic Editor uses the bee editor to add colored backgrounds, add multiple photos, embed videos, etc.
  4. Click Cookie & GET Parameters to choose which cookies or URL parameters should be recorded when the form is submitted. 
    • If multiple cookies or parameters map to the same field, arrange them by priority (highest first) to determine which value will be used.
  5. Click Payments to set up the payment gateway if there are any items for sale.

Form Questions

Use Form Questions to collect relevant data from students, such as including a phone number field to enable SMS/text opt-in functionality.

  1. Click the edit pencil to name the section.
  2. Click the plus sign to add additional fields, then check if the question is Required, Pre-populated, and/or Set Default Value .
  3. Use Edit Help Text to provide additional context around a question.
  4. Click Add Section to create a new section of questions. Click the edit pencil to change the name of the section, trash can to delete the section, and plus sign to add fields.
  5. Click Save Form.

Miscellaneous Information

General Settings

  • EMP does not recognize spaces in multi-line headers. Use HTML line breaks (<br></br>) to create new lines.

Inquiry Form Page 

  • You can customize the Inquiry Form Page title to better match your branding. If you leave this blank, it defaults to the Form Name in the General Settings.

  • The Meta description field can be edited and is useful for improving search engine optimization (SEO).
